Are you passionate about transforming lives through health and wellness? Crossroads YMCA is seeking a dynamic Community Health Director to pioneer innovative, data-driven health intervention programs. In partnership with local hospitals, you will be instrumental in enhancing the well-being of our community members, making lasting impacts through strategic leadership and compassionate engagement.

POSITION SUMMARY: This position supports the work of the Y, a leading nonprofit committed to strengthening community through youth development, healthy living, and social responsibility. Under the direction of the COO, the Community Health Director (CHD) is initially responsible for leading and implementing all daily operations associated with our Y Health initiative and will work towards implementation of a progressive array of evidence-based health intervention programs. This role includes but is not limited to providing support and guidance to participants and staff, coordinating classes, and ensuring that each program achieves quality performance outcomes. This hands-on position will work with clinically referred clients in achieving their goals in partnership with branch wellness teams. The CHD is strategically guided by the COO, YMCA operating principles, Association Leadership Norms and must operate in a manner consistent with our organizational values. The CHD is a vital member of YMCA leadership staff and a highly visible representative of the YMCA mission, core values and are accountable for knowing and properly executing all YMCA policies and procedures.

ESSENTIAL FUNCTIONS:

  1. Provide leadership towards safe, and effective operations of Y Health program.
  2. Serves as the primary point of contact for Y Health and Community Health programs.
  3. Lead portfolio by providing direction, leadership, support, and efficiency.
  4. Provide leadership to assigned service areas resulting in robust chart of works and growth.
  5. Champion program development, staffing and set standards that achieve programmatic outcomes that are reviewed and verified through audits, surveys, and regular participant feedback.
  6. Ensures the operational growth of the YMCA through program expansion, member recruitment and retention, space optimization, collaborations, partnerships, and third-party funding opportunities.
  7. Develop and maintain relationships with key partners, and other collaborative organizations and agencies related to portfolio.
  8. Ensure that all team members maintain required credentials and are aware of the expiration dates of their current certifications.
  9. Ensures accurate tracking of all applicable data points for each participant within the Community Health portfolio.
  10. Utilizes Electronic Health Record for referral processing, data collection and reporting.
  11. Ensures the implementation of association's risk management program to ensure safe facilities and programs for members, volunteers, and team members.
  12. Responsible for other duties as assigned.

QUALIFICATIONS:

  1. Bachelor’s degree in health, education or related field required.
  2. Minimum of two (2) or more years of professional experience in a wellness or healthcare field.
  3. Understanding of the nature and purpose of the YMCA and the respective roles of volunteers and team members in this organization.
  4. Ability to relate effectively to diverse groups of people from all social and economic segments of the community; track record of building authentic, constructive relationships with others.
  5. Microsoft Office experience and the ability to quickly learn EHR software is required.
  6. CPR/AED/First Aid and New Leader Orientation required with 30 days of hire.
  7. YMCA Organizational Leader certification preferred.

WORK ENVIRONMENT & PHYSICAL DEMANDS:

  • Visual, auditory, and verbal ability to communicate effectively.
  • Must have high level of alertness, concentration, and initiative.
  • Ability to lift 50 lbs. and stand, walk, or sit for extended periods of time; occasionally stoop/bend.
  • Maintain a neat and professional appearance at all times.
