How will this role have an impact?

The Population Health Manager helps clients improve health care quality and achieve ACO Shared Savings. The Population Health Manager achieves these aims by carefully tracking performance metrics, serving as a project manager, and training clients to effectively implement Signify Health Foundational Programs such as team-based care, annual wellness visits and hierarchical category coding. The Population Health Manager facilitates monthly calls with primary care practices, builds strong client relationships and identifies opportunities where additional subject matter expertise should be deployed.

Essential Duties and Responsibilities : To perform this job successfully, the Population Health Manager must be able to perform the following satisfactorily; other duties may be assigned.   • Tightly manage key performance indicators for assigned primary care practices.
  • Deliver and coach clients on Foundational Programs including patient messaging, same day visit processing, standing orders, pre visit planning, team based documentation, coding and documentation fundamentals, quality outreach campaigns, annual wellness visit workflows and use of face sheets.
  • Host and conduct monthly Roadmap calls with clients to support the implementation of population health priorities as determined in Steering Committee.
  • Provide project management and oversight to guide client's ACO work.
  • Serves as first escalation point for client questions and concerns.
  • Effectively communicates business and technology issues as well as solutions.
  • Build and maintain strong organizational relationships at Signify as well as within client organizations.
  • Integrate population health software (Signify Coach) into client interactions and assist clients to optimize their Coach utilization to meet ACO goals.
  • Gather data and track relevant performance metrics. Use this data to drive client discussions around improvement opportunities and progress towards goals and objectives. Escalate concerns to manager and regional team in a timely manner.
  • Document client progress and barriers and connect clients with SMEs as needed
  • Access and promote resources provided by subject matter experts on quality reporting, promoting interoperability, ACO compliance, value-based payment policy and other related programs.
  • Attend weekly regional huddles as well as client facing events like Quality Improvement Workshops and Symposium
  • Identify and promote Signify training content related to a specific clients' needs.
  • Holds client accountable to goals set in Steering Committee related to health care quality and ACO shared savings.
  • Stay current on evolving ACO program requirements from CMS as well as Signify Health.

Education/Experience:

  • Bachelor's degree from an accredited university or college is required.
  • Minimum two years' experience in provider clinic, health system, account management, client services or consulting.
  • Previous experience in project management preferred.
  • Previous experience working with Accountable Care Organizations preferred

The base salary hiring range for this position is $80,000 to $87,000 . Compensation offered will be determined by factors such as location, level, job-related knowledge, skills, and experience. Certain roles may be eligible for incentive compensation, equity, and benefits.
