Reports to: Assigned Provider and Director or Practice Manager
Department: Sweeny Community Hospital Clinics- Population Health
Summary/Objective: Provides basic clinical care to clinic patients under the direction of a registered nurse or eligible provider.
Essential Functions:

1. Contributes to the assessment of patients by collecting data for analysis by an RN, PA, of Physician
2. Administers medications and observes patients for adverse reactions to medications or treatments
3. Educates patients/families about disease treatment plan including self care post discharge, preventive and holistic health needs, available resources, and follow up care
4. Documents delivery of health care and nursing process in accordance with policies and procedures in a timely, accurate, and consistent manner
5. Reports abnormal health data
6. Coordinates and delegates the delivery of patient care provided by selected members of the healthcare team
7. Coordinates and delegates the assignment of routine tasks needed in order to keep the clinic operational to selected members of the healthcare team
8. Facilitates coordination of services with the non-medical clinic staff to obtain optimum clinic flow and increase patient satisfaction
9. Acts as a resource within the clinic to other staff members in regards to patient care
10. Acts as a resource for facilitating communication when necessary
11. Assists in the development and/or delivery of orientation and/or education processes for patients, families, and staff
12. Participates in monitoring and logging data for Process Improvement practices as assigned
13. Arrange and restock examining rooms with supplies, medications as required, instruments, and equipment
14. Explain and prepare patients for examinations as well as assist the providers during the exams
15. Purchase and maintain surveillance of expiration dates of supplies, medications and equipment
16. Collect and prepare laboratory specimen and/or perform basic laboratory tests on the premises as well as assistance with maintaining up to date CLIA certificates
17. Call in, fax, or otherwise assist Pharmacies with the appropriate filling of prescriptions ordered by provider
18. Dispose of contaminated supplies per protocol
19. Maintain office workspace and exam rooms in a neat and orderly fashion
20. Accurately and appropriately document the care given to the patient on the paper chart or in the EMR
21. Respect and maintain the privacy and dignity of patients and assure confidentiality of protected health information
22. Participate in staff and educational meetings
23. Facilitate referrals to other providers and share and obtain needed information as guided by HIPAA Regulations
24. Facilitate communication between the Patients, the Office Receptionist and the Provider
25. Maintain office quality control and temperature logs
26. May be required to float to other provider clinics
27. Performs other duties as assigned

Work Environment:
This job will require exposure to communicable diseases. This role routinely uses medical assessment equipment including sphygmomanometer, stethoscope, thermometer, scale, EKG machine. This role also routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets, fax machines, and other equipment as needed.

The physical demands described as follows are representative of those that must be met by an employee to successfully perform the essential functions of this job.
While performing the duties of this job, the employee is regularly required to talk and hear. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus. This position requires intermittent physical activity, endurance, ability to assist, support and position patients. Duties regularly require standing, walking, bending, kneeling, stooping, pushing, pulling and crouching as well as lifting and/or moving up to 50 lbs.
Position Type/Expected Hours of Work
This is a full-time position, and core hours of work are Monday through Friday 8 hours per day from 8:00 am – 5:00 pm with no overtime. Position requires work schedule flexibility based on needs of the facility.
TRAVEL
Occasional travel is expected for this position due to educational requirements.
Requirements
Required Education, Experience and License
High School Diploma
Licensed Vocational Nurse credentialed from the Texas Board of Nursing
BLS Certified from the American Heart Association
Preferred Education, Experience and License
At least 2 year(s) of experience working in a clinic environment
