Responsibilities: Sherman, TX Location
- The Hospice Registered Nurse (RN) will conduct a detailed assessment of the patient & family during the initial start of care.
- Develop a plan of care with the collected start of care data.
- Case manages patient care episodes and works one on one with patient/ family to improve health and stability.
- Complete clinical notes, orders, and charting in a timely manner.
- Report and consult with physicians and care team regarding patient care and goals.
Qualifications:
- Current Texas RN License
- Valid Driver's License
