Key Responsibilities
The Triage Coordinator is responsible for the coordination of all after hours calls, after hours orders, and after-hours communication with on-call provider, patients, facilities, and families for Physician Services. Responsibilities are completed with limited supervision and after hours. This position requires initiative, creative problem resolution, time management, excellent phone and electronic communication skills, service excellence and a desire to work within a practice that centers around and with a geriatric population.
  • Communicates significant findings, problems, changes in condition or environment to the primary care providers and other personnel involved with patient care
  • Performs well under stress and in emergency situations
  • Assumes primary responsibility for “after hours” including planning, coordinating, implementing, and assisting with the evaluation of the plan of care for patients, families and facilities
  • Answer telephone calls after hours and responds to any concern or change in patient condition. Provides support and direction to agency staff, health care professionals patients, and families related to health care concerns
  • Follows through with the coordination of provider orders. This may include but is not limited to coordinating home healthcare or hospice service, delivery pharmacy, portable diagnostics, caregiver support services and laboratory services
  • Responsibility for communicating with patients, families and facilities after hours, triaging patient needs and communicating this information to the on-call providers in a timely manner
Required Qualifications
  • High School Diploma or GED
  • Minimum one (1) year of experience in medical field, medical office experience or related position
  • Ability to operate computer equipment, Fax and photocopy machines necessary
  • Must be proficient in Microsoft Office Suite, including Outlook, Word, Excel, and PowerPoint
Desired Qualifications
  • Associate Degree preferred
  • Preferred C.N.A. certification or MA credentialedPrefer experience with geriatric population and or caregivers
Location
  • Home Office: 4760 Fashion Square Blvd, Saginaw, MI
Hours
  • Office Hours: 11:00 am - 8:00 pm 3 days per week plus weekend rotation every other weekend.
