Hospice Liaison Opportunity in Lawrenceville, GA

Titan Placement Group is currently seeking a compassionate and experienced Hospice RN Case Manager to join a reputable healthcare team in Lawrenceville, GA, located in one of Atlanta's fastest-growing counties, Gwinnett County. Known for its bustling city life, thriving communities, nationally recognized school system, and beautifully maintained natural spaces, Lawrenceville offers an inviting environment for healthcare professionals.

Salary and Benefits:

• Competitive salary starting at $80,000 - $100,000
• Expected Annual Compensation ($120,000 to $160,000)
• Incentives start after 7 referrals
• Up to $450 per referral
• Health Insurance: Employer covers 75%
• Dental and Vision Insurance provided
• Paid Time Off: 19 days in year one
• 6 paid holidays
• Mileage Reimbursement: $0.50 per mile
• Cellphone Allowance: $60 per month

Responsibilities:

• Monday – Friday
• Reports to the Executive Director
• 7 Referrals a Month Quota

Requirements:

• Hospice or Medicare Home Health Experience Required – 2 years
• Liaison, Sales, or Business Development Experience Required
