Description Of Duties And ResponsibilitiesMaintains confidentiality of all information regarding clients, staff, physicians and GCLC obtained as a result of employment. Primarily responsible for appropriate documentation of service delivery, billing for services, and teaching of independent living skills as developed in compliance with contractual, Medicaid, Medicare, Department of Mental Health and GCLC requirements. Documentation of services shall be in the form of progress notes including the beginning and ending time, the duration of billable service, the client’s daily goal and description of the participation that day. The implementation and service delivery will include but not be limited to, personal budgeting and financial management, basic living skills, coping skills, personal hygiene, comparative shopping, stress management, etc. Responsible for providing transportation services as needed for clients. Requisite, supervise and facilitate recreational and social activities. Responsible for ensuring the safety of all clients and leading meaningful activities within the program milieu. These activities may include but are not limited to: therapeutic socialization, classes, community meeting, and other such events that together constitute the schedule of the integrated program model. Must be able to demonstrate the ability to assist clients with disabling mental illness to achieve their maximum level of community integration and self-reliance. Assist client to learn to compensate for the effects of the symptoms of the illness through the development of new skills and a supportive environment and to counteract the effects of secondary symptoms by restoring a sense of confidence. Responsible for other duties as assigned by the Community Support Program Supervisor or the Clinical Director of Residential Services.
QualificationsMust be at least 18 years old. Bachelor’s Degree in a human service field from an accredited university and one-year relevant experience with the target population; or Associate’s degree and a minimum of two (2) years of experience working with the target population; or High School graduate of General Educational Development (GED) test and a minimum of three (3) years of experience working with the target population or certified as a Peer or Family specialist.
Minimum staff training requirements include twenty (20) hours of documented training and/or continuing education within the first 90 days of employment and must maintain twenty (20) hours of continuing education annually. Must obtain CPR and Basic Life Saving Techniques within the first 90 days of employment. Must pass CYFD/FBI fingerprint clearance check to maintain employment. Must pass and maintain a Driving Record clearance in accordance with GCLC policies and procedures. Preference given to qualified bilingual (Spanish/English) candidates.
