Raleigh Oaks Behavioral Health is Hiring a Lead Case Manager Near Garner, NC
About the Job:The Lead Case Manager is responsible for the day-to-day effective delivery of patient care through case management including the overall discharge and aftercare planning needs and placement of the patient. Assesses, plans, implements, coordinates, monitors and evaluates services to meet an individual’s needs during treatment.
Roles and Responsibilities:
- Assures clients/patients/residents meet care criteria from admission through discharge and patient care is delivered in the most cost-effective manner.
- Uses pre-established guidelines with criteria, reviews initial clinical information and initiates admission certification with external review entities.
- Maintains knowledge of clinical information by concurrent review of documentation, participation in staffing, attending case conferences and collaborating with other members of the treatment team to maintain certification with external review entities.
- Monitors medical record documentation using knowledge of established guidelines to promote compliance and further ensure quality for retrospective review by external review entities.
- Makes appropriate referrals to physician advisor as needed.
- Acts as liaison with facility and MCO to assure that all aspects of treatment are fully documented.
- Assists in identifying other options for care when necessary.
- Assures a seamless flow of information between levels of care by collaborating with the admissions department, clinical and financial departments, and patient’s significant other/guardian to provide a smooth transition.
- Assures coordination of resources with other community services and agencies.
- May oversee daily operations of the team and assign staff as clinically warranted.
- Bachelor's degree in Social Work, Counseling or related human service field required.
- Three or more years' experience working in a behavioral health setting required.
- Previous experience with the patient population served by the facility preferred.
- One or more years’ experience in case management required.
