As a Care Transition Nurse (CTN), make a lasting impact by coordinating quality home-based services and guiding patients to the best care.
Collaborate with physicians, hospitals, and care facilities to provide coordinated and quality home-based services. Be the beacon of support and guidance in our healthcare landscape.
Assist discharge coordinators in coordinating home care services, empowering patients to make informed choices for their wellbeing.
Provide patients and families with compassionate education on home care regulations and services.

Qualifications- Graduate of an accredited school of nursing.
- Current RN, LPN, MSW, or BSW license to practice in the state serviced.
- Hospital and/or homecare nursing experience.
- Experience as a Care Transition Nurse or similar role, sales or marketing, or customer service.
