Catholic Home Care manages home care services for approximately 3,000 patients daily across Long Island and Queens. Specialized services include skilled nursing care, physical and occupational therapies, medical social work, maternal-child and pediatric services, home health aide assistance, nutritional counseling, pastoral care, infusion services, telehealth, palliative care, wound care and behavioral health services.
The Speech Language Pathologist assesses, plans, develops and provides the patient or responsible caregiver a home health plan of care for speech language pathology and identifies patient needs for other disciplines and services. Coordinates and evaluates the plan of care with all other disciplines and physicians for an assigned caseload. Instructs the patient or responsible caregiver on the projected care plan and expected outcomes as directed by the physician's treatment plan.
Requirements:
The pay for this position ranges from $90,000-$105,000 per year.
This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job and it does not include the value of benefits.
