Job Summary:
Provides case management services to victims of domestic violence and sexual assault. Responsible for maintaining accurate data for reporting purposes. Completes monthly, quarterly and annual reports.

Responsibilities:

• Facilitates client’s access to community resources, including low-income housing, Medicaid, food stamps, food, clothing, educational programs, life skills training, mental health and medical services
• Assists with completing written service plan for crisis shelter residents including measurable goals and action steps
• Coordinates and monitor services, including comprehensive tracking of client activities in relation to service plan
• Evaluates effectiveness of service plan and progress made by client
• Participates in weekly group meetings with shelter residents
• Conducts morning meetings (Monday-Friday) with each shelter resident to track daily appointments and progress toward accomplishment of their designated service plan, goals and action steps
• Provides backup for completing intake assessments for crisis shelter clients within 24 hours of admission
• Assists with and make recommendations for immediate relocation of ineligible shelter residents
• Works in conjunction with all other program components and staff to ensure effective communication of all client concerns, recommendations, and needs
• Completes all paperwork as needed to assist clients with rapid re-housing
• Provides client transportation as needed to community locations, various agencies, and other destinations to achieve their goals
• Facilitates weekly group with victims of domestic violence and sexual assault
• Travels to community locations, various agencies, and other destinations as needed
• Assists with making appropriate client referrals for continued counseling upon discharge from crisis shelter
• Attends and participates in weekly case management team meetings

Requirements:
Minimum requirement of a Bachelor's degree in social work or related field. Minimum two (2) years of related experience. Worker must have knowledge of community resources and have excellent organization and computer skills. Must have good judgment and be flexible in job responsibilities. Child Abuse Registry and criminal background checks and fingerprinting will be required and have no violations. Position requires reliable transportation, valid driver's license, and current automobile insurance.
