University Hospitals is Hiring a Social Worker 2 - Peds ED Near Cleveland, OH
DescriptionPosition Summary/Essential Duties: Collaborates with all members of the health care team, patient and family (or significant others) to coordinate timely and efficient assessment of psychosocial and discharge planning needs and formulation of patient and family centered discharge plans. Serves a resource to others in work-team and department.
Provides age specific care to patients/families in all age groups.
Develops, documents and implements a comprehensive plan based on identified psychosocial needs and/ or discharge needs.
Develops plans with attention to individual patient and family goals
Provides linkage to community resources and UH network services to facilitate optimal care
Communicates/collaborates with the work team using a multidisciplinary approach.
Provides guidance/ consultation to peers/team regarding routine psychosocial issues and /or discharge planning issues.
Provides supervision to social work interns if LISW eligible (completion of supervision hours).

Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients. Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace.
QualificationsPosition Requirements
Two (2) years’ experience in a hospital setting desirable
Computer skills
Theoretical and clinical knowledge regarding health care delivery
Multi-tasks and prioritizes work
Communicates effectively with persons of various backgrounds
Demonstrates strong interpersonal skills and excellent communication skills, both written and verbal
Exhibits strong organizational skills and work ethic
Demonstrates a professional demeanor when responding to patients/customers
Education RequirementsMaster’s degree from an accredited School of Social Work License RequirementsLicensed by the State of Ohio Counselor, Social Worker, Marriage and Family Therapist Board at the LSW level; Attainment of LISW status is encouraged within one year of eligibility. Preferred QualificationsPrevious experience working with pediatric emergency situations is strongly preferred. Recent work with pediatric trauma is also preferred. Will be performing psych and forensic assessments, so candidates with mental health evaluation and acute/crisis experience is preferred. Schedule is (3) 12 hour shifts,mainly 11:00am-11:00pm and 1:00pm-1:00am; Mon-Fri with rotating weekends and holidays. Physical Demands
