CenClear , a provider of Assertive Community Treatment (ACT), is seeking dedicated and compassionate individuals to join the team. The ACT program is a community based delivered service, which uses an evidence-based practice aimed at improving outcomes for individuals with severe mental illness.

Location

  • Travel within Blair, Clearfield, Elk, Huntingdon and Jefferson counties to multiple locations, which may occur in the office, community, or clients' homes.

Compensation

  • This is a full-time position
  • 40 hours per week, 52 weeks per year (includes non-traditional hours)
  • Compensation for time and mileage based on reimbursement policy

Qualifications

  • Master’s Degree or above from an accredited college with a major in chemical dependency, psychology, social work, counseling, nursing (with a clinical specialty in the human services), or other related field which includes a practicum in a health or human service agency, licensed eligible
  • Must obtain the following clearances prior to hire: PA State Police Criminal History, PA Child Abuse History, FBI Criminal History, and Mandated Reporting Certificate

Responsibilities

  • Provide consistent, high-quality, recovery-oriented, and intensive, individualized and comprehensive treatment in line with agency philosophy and best practices.
  • Promote a physically and emotionally safe, welcoming therapeutic environment.
  • Work cooperatively with the ACT team to meet the individual needs of clients.
  • Coordinate referrals and exchange of records as needed with appropriate release of information.
  • Develop individualized, recovery-oriented treatment plans that promote overall physical and behavioral health well-being.
  • Collaborate with clients on goals for discharge and aftercare planning to support ongoing growth and change.
  • Maintain up-to-date medical records as specified by state/program regulations.
  • Complete all documentation within the electronic health record (EHR).
  • Participate in required in-house and appropriate out-of-house trainings.
  • Attend staff meetings and participate in formal group supervision and formal individual supervision twice per month.

About the Program

ACT provides highly individualized and comprehensive services to individuals based on their needs (outpatient therapy, employment, housing, etc.) ACT recipients receive the multidisciplinary, around-the-clock staffing of a psychiatric unit, but within the comfort of their own home and community. The ACT program is a community based delivered service, with non-traditional hours. To have the competencies and skills to meet a client's multiple treatment, rehabilitation, and support needs. ACT team members are trained in the areas of psychiatry, social work, nursing, substance abuse, and vocational rehabilitation. The ACT team provides these necessary services 24-hours-a-day, seven-days-a-week, 365-days-a-year.
