Position Overview:

As a Case Manager, you will play a crucial role in supporting our clients through their recovery journey. Your responsibilities will include:

• Meeting with clients to assess their needs and create personalized treatment plans.
• Collaborating with an interdisciplinary team to provide comprehensive care.
• Connecting clients with community resources and helping them set and achieve their goals.
• Contributing to the development and implementation of new programs and services.
• Supporting clients in navigating the complexities of their treatment and recovery.

Qualifications:

• High school diploma or GED required.
• Valid driver's license preferred.
• Strong communication skills, both written and interpersonal.
• Basic technology skills and the ability to work with diverse populations with cultural competence.
• Ability to work effectively within a team and independently.
• Willingness to learn and grow within the role.

Schedule:

• Full-time (40 hours per week).
