SUMMARY OF JOB:

Performs medically prescribed occupational therapy treatments to restore function and prevent disability following injury.

RECRUITMENT REQUIREMENTS:

* Graduate of an OTA approved and accredited college of Occupational Therapy.

* Excellent oral and written communication and interpersonal skills.

* Demonstrate competency in applying principles, methods, materials and equipment used in occupational therapy.

* Ability to assess patient needs and develop and implement a comprehensive plan of care.

* Ability to function as a member of an integrated Healthcare work team.

* Able to travel to patient's home within service area.

* CPR required.

* Able to demonstrate professional procedures in the home care setting.

* Able to use office equipment; i.e. tablet, cell phone.

* Ability to work flexible hours, with weekends and holidays included.

LICENSE AND REGULATORY REQUIREMENTS:

* Valid driver's license.

* Current license in the state of practice.

CORE RESPONSIBILITIES/ESSENTIAL FUNCTIONS:

* Performs evaluation diagnostic test and skilled occupational therapy procedures on patients.

* Instructs patients, caregivers and other medical personnel regarding safe performance of procedures and other supportive activities.

* Collaborates with physician to ensure that orders are obtained.

* Participates in the development and re-evaluation of the patient plan of treatment in collaboration with and under the approval of the physician.

* Collaborates with interdisciplinary team across the continuum of care and functions as case manager as needed.

* Demonstrates sensitivity to cost containment requirements.

* Documentation accurately reflects patient status and needs and is submitted within 24 hours of the visit. This includes a good working knowledge of OASIS assessments.

* Exhibits effective communication skills with patients, caregivers, physicians, managers and other health care workers.

* Requests for insurance authorizations are completed on a timely basis.

* Participates in team meetings and staff education requirements and integrates new information into clinical practice.

* Participates in accreditation survey preparedness and related corrective action follow up.

* Is aware of and participates in quality improvement initiatives and infection control plans as necessary.

* Understands and complies with all local, federal and regulatory body requirements.

PHYSICAL AND ENVIRONMENTAL REQUIREMENTS:

See attached physical capacities form for Direct Patient Care Provider. Potential exposure to extreme heat, cold, hazardous substances, cigarette/cigar smoke, contagious diseases, noise, pets, and varying weather conditions.
