Always Best Care is currently hiring a full time RN for Non Medical In Home Care.
The RN Supervisor provides oversight to patient services- including assessment, admission care planning, supervision and discharge of all clients The RN supervisor coordinates care with other professionals and obtains the physician authorization as indicated. The RN supervisor participates in the orientation and training of all clinical staff, validates skills and provides continued in-service training and clinical staff supervision. The RN supervisor participates in the ABC Performance Improvement Plan including quarterly record review, complaint investigation and incident management. The RN supervisor develops new policies as needed, in accordance to NC Board of Nursing Practice Guidelines and standards of Care and submits to the Director for final review and governing body approval.

* Primary Responsibility
* Complete initial in home assessments of referred clients. To include at a minimum:
* In home Assessment of client and service needs and adequacy of home environment
* Obtains Medication and treatment orders and/or PACT authorization
* Discusses and explains client’s rights and responsibilities with clients at admission
* Develops, supervises and updates a client plan of care to include type of services needed, frequency of services needed, days services are needed, and time of day services are needed. Delegates tasks to nurse aides for in home aide services.
* Provides explanation to potential client of the scope of services that this agency provides and which services this agency will or will not be providing to them (according to the individual client care plan needs) upon approval from there primary/attending physician.
* Completes and records assessment, client’s plan of care for physician’s approval, including PCS / PACT or CMS 485 and 486 and physicians order plan of treatment
* Return upon completion all documents to agency premises.
* Conducts client in home Supervisory visits at least every 90 days and documents such findings in 5 days. Return all completed forms to agency premises per policy. Completes Nursing Supervision per standards, including evaluations of all clients to include at a minimum.
* Any and all changes to be noted in clients current plan of care
* Client’s condition including physical, mental, emotional, and living condition
* Quality of service that is currently being provided by the client by this agency.
* Document any comments or complaints from client or client’s family.
* Document any items requested by client or that you may feel that the client may be in need for assistance such as: walkers, wheel chairs, shower seats, hospital beds, diabetic shoes, etc. complete any items request form for the attending physician to complete and approve.
* Document any safety concerns regarding client’s current condition die to mental, physical, emotional, or living conditions. Document any suggestions in resolving the concern
* Return all documentations to the agency premises with in 5 days.
* Reports to the agency director and records any and all complaints made to RN by any persons or family members of persons receiving services from this agency, or any persons providing services to other persons of this agency to the owner director immediately. These complaints will be documented in writing dated and signed by RN and the agency director. Follow up or actions taken to resolve the issue will be documented dated and signed by all persons involved with in a reasonable amount of time.
* Completes quarterly record review; participates in PI Plan, including providing incidents, complaints and participation in annual agency evaluation.
* Provides in home staff supervision and validation in home staff skills
* Establishes and maintains all skills evaluation and competency test for all active and potential in-home aides providing.
* Competency of each in home aide to be documented before being assigned any job assignments.
* Quarterly evaluations of all in-home aide job performance.
* Annual evaluation to be conducted in the client’s in-home aides.
* Documentation of each evaluation and all competency tests are to be returned to agency premises and placed in personnel files.
* Continuous availability during the hours for which the agency will be providing services to clients, by means of a cell phone that will be issued to you by the agency director. All contacts will be made by home phone contacts, cell phone contacts, cell phone contacts, person-to-person meeting, etc.
* Provides assistance in overseeing those agency policies is being followed by all agency staff including owner/director.
* Provides assistance in other areas that the director may require your advice or guidance.
* Develops procedures for new care activities not listed on current care. Determines with BON care tasks appropriate. Researches standards of care to develop procedures.
