Epic Nursing Services is Hiring a Nurse Care Manager Near Braintree, MA
RESPONSIBILITIES
- Complete and coordinate all applicable clinical assessments and re-assessments and clinical evaluations.
- Complete all required prior authorization and track expiring authorizations.
- Develop and review on an ongoing basis each member's plan of care.
- Review the physician orders, medications and diagnosis.
- Monitor the health status of all members to ensure that all ordered services are properly delivered;
- Report changes in the health status of any member to the member's primary physician.
- Review direct care aide and caregivers' documentation as directed.
- Conduct on-site home visits with each member enrolled in accordance with the visit cycle policy.
- Complete a progress note corresponding with each on-site visit or encounter, or more often as the member's condition warrants.
- Submit semi-annual health-status reports to the member's PCP.
- Plan for and implement discharges and transition from the agency programs.
- Conduct an orientation for each Direct Care Aide or caregiver before they can begin care provision.
- Provide ongoing training to Direct Care Aides and caregivers on health and aging
- Contact with members primary care physician to obtain medical information and physician clearances
- Other responsibilities as assigned by the Agency.
- Must be a graduate from a School of Nursing.
- Must be licensed in the State as a Registered Nurse or a Practical Nurse.
- Ability to travel throughout Massachusetts, mode of transportation provided.
- Minimum two years of experience working with elderly or disabled individuals.
- Excellent organizational, problem solving/planning and communication skills
- Demonstrated skills in Windows, Microsoft Word, Excel, Access and Outlook.
- Must be able to contribute to the quality of care being rendered through constructive communication with administrator and staff.
- Must have a criminal background check.
- Must have a current CPR certification.
