UHS hospital is Hiring a Remote Peer Support Specialist

Peer Support Specialist, UHS New Horizons United Health Services is seeking a Peer Support Specialist to join our Chemical Dependency department at UHS New Horizons in Binghamton, NY. This position is benefits eligible. Shift Day Rotational Hours per week: 40 Salary range: $16.35 - $21.49 per hour, depending on experience. Overview The Peer Support Specialist provides peer support, information, and advocacy to individuals diagnosed with a serious and persistent relevant illness, in order to assist them in regaining control over their own lives and recovery process and integrating into the community. They engage with patients hospitalized at UHS Hospitals. Qualifications OUTREACH Conduct regular outreach targeted at reaching people at risk for or living with relevant health issues or at risk of acquiring them. Outreach activities will include prevention education or treatment education including accessing external services, linkage to care services, and providing applicable referrals/referral follow up. PATIENT NAVIGATION Work closely with patients to access, complete treatment and assist with any transitions to outpatient services. Assist with case management including assessing participants for immediate needs that pose a barrier to treatment such as lack of insurance, transportation issues, housing, food insecurity, etc. as necessary. Escort patients to appointments as needed. Assist with medical appointment and lab appointment reminders in combination with other members of treatment team. Educate patients to benefits of individual and group therapy. Assist with completing intake paperwork prior to attending appointment after discharge as necessary. Educate to policies and processes of the applicable clinic; what to expect. DATA COLLECTION Maintain accurate documentation of program activities. Document all patients’ progress in electronic medical record as required.
