Description:

Intake nurse needed for large hospice organization. Will be responsible for referral coordinating, verifying and entering physician's orders, identifying level of care required by patient, review documentation to determine facility placement, coordinate orders with pharmacist, works closely with referring hospitals, physicians, skilled nursing facilities, patients, and families.

Monday Friday (rotating shifts: 8:30-5:00pm / 10:00-6pm)

One schedule weekend every 8-12 weeks, and rotating monthly on-call

Additional Skills & Qualifications:

LPN/RN licensed in State of Utah

1 year experience preferably in killed nursing facilities

BLS certification
