Altius Home Health is a Colorado based ACHC accredited home health agency, seeking a part time/PRN Registered Nurse to provide care for primarily adult patients in their homes in Denver Metro Area. This position is PRN/part-time with the possibility of full-time, if desired by the clinician.
The perfect candidate will be responsible for the following:
1. Evaluates patients referred for home care to determine appropriateness for admission (Start of Care/OASIS).
2. Establishes a home care plan including assessment of the appropriateness of the requested services.
3. Assures that Agency is ready to meet the patient’s needs by collaborating with Agency personnel on a regular basis.
4. Explains Agency’s policies and services to the patient and responsible family members during initial home care assessment.
5. Completes documentation accurately, and submits to agency office in a timely manner.
6. Establishes and promotes an ongoing communication with Agency personnel.

This is a part-time or PRN position that can turn into full time, Monday-Friday and occasional weekends
The schedule can be flexible, based on your availability ( mornings, afternoons, evenings, weekdays or weekends, if preferred).
Pay:
$50- $100 per visit depending on the visit type
