The Role of a Patient Care Coordinator in Waynesboro, GA

Job Description : As a Patient Care Coordinator, your responsibilities include :

• Being accountable for managing and leading the assigned team's patient census, focusing on direct management of the Clinical Support Specialist and Field Clinicians.
• Ensuring effective communication and coordination of care needs reported by case managers.
• Updating schedules and maintaining continuity of care for patient episodes, while ensuring quality care is delivered by the clinical care team.
• Facilitating case conferences and taking responsibility for follow-up actions.
• Providing coaching and mentorship to clinicians and CSS role, including addressing development needs and monitoring role-specific key performance indicators (KPIs).
• Acting as the primary point of contact for support and problem-solving to mitigate personnel concerns within the assigned team.
• Supervising and directing team members' work, including assigning and scheduling visits in a timely manner.
• Managing the site in the absence of the Clinical Manager, Executive Director, or Administrator.
• Overseeing clinical, administrative, and recordkeeping functions.
• Responding to phone referrals and inquiries professionally and compassionately.
• Participating in on-call nursing schedule and providing skilled bedside nursing care in patients' homes.

Requirements for Qualified Candidates :

• Must be a registered nurse with a current nursing license in the practicing state.
• Must possess and maintain CPR certification.
• Previous experience in a home care setting with at least two years of management or supervisory experience is preferred.
• Proven ability to work effectively within an interdisciplinary setting.
• Must hold a valid driver's license and have a properly insured and well-functioning automobile.
• Knowledge of professional standards and practices, Medicare Conditions of Participation, and federal, state, and local regulatory requirements.
• Demonstrates excellent nursing judgment, observation skills, and communication abilities.
