- Assisting the resident Physical Therapist in all aspects of member therapy and care plan needs.
- Providing interventions based on member-specific needs, including therapeutic exercise, gait training, ambulation and other physical assistance.
- Monitoring and documenting member treatments and interventions, ensuring all members of interdisciplinary teams have access to this vital information.
- Updating Plans of Care based on member progress and evolving needs.
- Identifying areas for individual and group improvements and enhancements, and implementing new procedures and processes under the supervision of the Physical Therapist.
- Developing group exercise programs for maintenance therapy members.
- Aiding in in-home treatment programs, including goal setting and exercise, plus family and caregiver education.
- Ensuring complete compliance with all Senior LIFE, state and federal regulations and best practices.
- Assessing member progress both in centers and in-home, and determining comprehensive courses of action to improve outcomes or maintain success.
