Alternative Structures International is Hiring a Rapid Rehousing Case Manager Near Waianae, HI

Position Title: Rapid Rehousing Case Manager

Status: Full-Time

Supervised By: Permanent Housing Manager

Primary Purpose:

This is a non-supervisory position located at Ohana Ola O Kahumana housing facility. Rapid Re-Housing (RRH) Homeless Program is offered to families to quickly exit homelessness and offered without prerequisites (such as employment, income, sobriety, etc) The program offers services tailored to the need of the household. RRH Case Manager will be the guidance and source of support to assist clients in identifying strengths and resources to avoid homelessness. The RRH Case Manager will assist families and engage in goal planning to help families with resources and skills that they need in order to achieve long term housing stability.

Essential Functions:

Provide low barrier to entry and ongoing program participation. Offer families immediate access to shelter without unnecessary prerequisites.

Complete intake documents and possess the ability to thoroughly assess participants' needs and identify housing, family, social, income, educational, employment, and legal barriers.

Maintain contact with all clients on a scheduled basis appropriate to their needs; ex: daily, weekly, bi-monthly, etc.

Assist clients as advocates for benefits, housing needs to include evictions, landlord negotiations and access to housing resources.

Creates permanent housing opportunities by developing an inventory of available housing units for eligible clients with varying price ranges.

Maintains landlord/rental management relations and is responsible for on-going assessment, service plan development, ongoing counseling and referrals for program-eligible clients.

Performs housing assessment and intakes to qualify clients per program specifications.

Inspects rental units and assists in conducting on-site behavior training including tenant safety education.

Provides life skills training where necessary as well as on-going assessment, service plan development, counseling and referrals for program eligible clients.

Create and maintain individualized residential plans that set goals, interventions and performance measures.

Ensure that all documentation is timely and accurate to meet funding/program requirements and HIS standards of documentation

Assist in increasing income and assets by screening and advocating for public benefits, TANF, SNAP, SSI, etc.

Provide the linkage to appropriate services in addressing participants' needs and barriers through referrals; ex: health, mental health, addiction, legal, employment, etc. Follow up on referrals to maintain and manage progress toward self-sufficiency.

Provide client training in areas of life skills, tenancy skills, financial literacy, personal hygiene, accessing community resources, etc

Provide individual support to help build self-esteem, motivation, achieve individual’s potential, health, family care, nutritional and financial planning.

Monitor and follow up with participants' progress through bi-monthly progress reviews and quarterly updates of program participants' service plan

Participate in monthly inspections.

Establish a positive relationship with client agencies to promote a continual enhancement to participants' self-esteem, education, employment, housing and family assistance through personal counseling or referrals to partnership agencies.

Maintain and update case management files on a regular basis. Document all client interaction.

Complete monthly reports and calendars.

Attend weekly case management meetings and other specified meetings/trainings as required

Provide quarterly reports to the Housing Program Manager

Assist with other projects and activities as assigned by the Housing Program Manager/Housing Director

Perform other duties as assigned.

Physical Demands:

Approximately 25% of the time the incumbent must bend, reach, lift or move up to 25 pounds, exposure to blood or bodily fluids, exposure to chemicals, and exposure to toxic fumes, work around water, work outdoors, drive and work evenings and weekends.

Work Environment:

This position typically requires work in a normal office environment.

Requirements:

A Bachelor’s degree from an accredited four-year college or university majoring in Human Development, Sociology or any related educational background and/or equivalent work experience is required.

A demonstrated understanding of the needs of homeless families and an ability to positively empathize with assigned families and the conditions they face. The ability to work in a diverse community and a strong knowledge of the Wai’anae Coast community.

Ability to communicate orally and in writing and to read, analyze, and interpret professional journals, technical procedures, and government regulations, write reports, business correspondence, and procedures.

Ability to effectively present information and respond to questions from groups, participants, and the general public.

Ability to add, subtract, multiply, divide in all measures, using whole numbers, common fractions, and decimals. Ability to compute rates, ratios, and percentages.

Ability to produce a family budget and give instructions to families with different abilities of understanding.

To perform this job successfully, the incumbent should have a working knowledge of Microsoft Outlook, MS Excel spreadsheet, MS Word processing and Internet Explorer.

The incumbent must have a current Tuberculosis (TB) Clearance and test negatively when drug tested. A valid Hawaii's driver license with proof of insurance, a current safety check and abstract are required.

Pay: From $40,000.00 per year
