Renviva is looking to hire a Social Worker to assist patients at our Upper Sandusky and Kenton locations.

Responsibilities include:

-Assessing, educating, and collaborating with patients on care planning.

-Master's degree, LCSW preferred will accept LSW.

- 1 year of clinical experience with Renal/Dialysis patients preferred/training offered.

Job Type: Contract

Pay: $25.00 - $45.00 per hour

Expected hours: 20 – 30 per week
