If that sounds like you, apply now and become part of our team at Giving Home!We are looking for a compassionate and skilled Licensed Practical Nurse (LPN) in Tonopah, NV to administer patient care in accordance with a physician established care plan. This role is a 1099 contract opportunity, with care being provided at the patient’s residence. Patient needs range from weekly hours to 24-hour care. This allows for multiple scheduling options, from short visits on a weekly basis to full 12-hour shifts or a blend of both. Patients range from high acuity to those who only need basic medical management.Once our patients are accepted into the program, there is no expiration for this benefit, which allows our team of caregivers to build lasting relationships with their patients for years to come.You’ll get to advocate for the patients and work one-on-one to provide them with the care that they need. This role will collaborate with CNA’s, HHA’s, Case Managers, and the patient’s physicians. #PIQRN ResponsibilitiesExecute physician prescribed plans of careComplete compliant documentation of the care provided in the system of recordManage and administer prescribed medication, treatment, and therapiesConduct patient assessments, coordination of carePerform various activities associated with daily livingMonitor vitals and GI intake and outputAssess skin integrity and administer wound careMonitor for changes in the patient’s condition such as weight loss/gain, self-care abilities, and indicators of disease progressionCommunicate with family members, physicians, case managers, and other prescribed individuals regarding the health of the patientsProvide education and training to the patient, their family, and/or the Home Health AideWork with patients to be done one-on-one in the patients home to provide for a comfortable environmentCollaborate with and supervise Certified Nursing Assistants and Personal Care AttendantsPerform other personal care services as necessary to meet the patient’s needsMinimum QualifactionsEducation, licensing, and certification: graduate of an accredited college or university, Associate or Baccalaureate School of NursingCurrent State or Compact License as a Licensed Practical NurseTB test and physical examinationCurrent CPR certificationMaintain required insuranceGood interpersonal skillsHand-eye coordination and manual dexterityAbility to stand or walk for prolonged periods, with the ability to lift up to 50 lbs and move patientsAbility to operate under stressful conditions and make quick decisionsMust have dependable transportation and be willing to commute to the patient’s homeAbility to provide basic medical management up to medically fragile end of life carePreferred Qualifications1 years of experience as an LPN in a clinical or home health setting.Ability to assist with two person assistIn order to ensure the life-changing care our patients require, we know that we must have a team of exceptionally caring and passionate individuals who are committed to advocating for and empowering those in need.
