Overview:
We are seeking a dedicated Registered Nurse Case Manager to join our healthcare team. The ideal candidate will have a strong background in acute care, ER, infectious disease care, primary care, hospital medicine, PICU, and medical-surgical settings.
Duties:
- Provide comprehensive nursing care to patients with various medical conditions
- Conduct patient assessments and develop individualized care plans
- Collaborate with healthcare professionals to ensure coordinated and effective patient care
- Monitor patient progress and update treatment plans as needed
- Maintain accurate medical records and documentation
- Educate patients and their families on health management strategies
Experience:
- Previous experience in acute care, ER, infectious disease care, primary care, hospital medicine, PICU, or medical-surgical settings is preferred
- Strong background in patient assessment and care planning
- Proficiency in medical records management
- Excellent communication and interpersonal skills
- Current RN license and BLS certification
