Pinnacle Home Care is Hiring a Home Health Medical Social Worker (MSW) Near Tampa, FL
Position Summary: As an MSW, you'll provide medical social services to clients in their homes under the supervision of the Director of Clinical Services or Clinical Manager. This includes initial evaluations, assisting in care plan development, and providing support to clients and families.
Responsibilities
- Perform initial evaluations and assess clients' psychosocial status.
- Develop and implement client care plans related to medical social work.
- Observe, record, and report changes in clients' conditions.
- Consult with physicians and participate in care coordination.
- Participate in staff development activities and in-service education.
- Engage in discharge planning and continuous self-development.
- Master’s Degree in Social Work from a school approved by the Council of Social Work Education.
- Licensed as a clinical social worker (LCSW) in Florida.
- Current BLS Certification.
- Two years of experience, with at least one year in a healthcare setting.
- Valid driver's license and insured automobile.
