We are seeking a compassionate and experienced Registered Nurse to join our dedicated homecare team in Broward County. As a Homecare Registered Nurse, you will play a key role in providing high-quality, personalized care to our clients in the comfort of their homes. Supervisory role to all homecare aides.The Registered Nurseis a qualified professional responsible for the planning, coordination, and assurance of quality care services to patients and their families.

Responsibilities:

Patient Assessment and Care Planning:

• Conduct comprehensive patient assessments to develop and maintain individualized care plans.

Quality Assurance:

• Implement and adhere to quality assurance programs to ensure the highest standards of patient care.
• Monitor and evaluate patient outcomes, adjusting care plans as necessary.

Client Advocacy:

• Advocate for patients' rights and preferences, ensuring respectful and person-centered care.
• Collaborate with interdisciplinary teams to address the holistic needs of clients.

Communication:

• Maintains ongoing communication with patient, their families, physicians, other disciplines, and office staff.
• Provide education and support to employee aides, clients, and families on health management.

Documentation:

• Ensure accurate and timely documentation of patient assessments, care provided, and changes in condition.
• Adhere to regulatory and agency documentation requirements.

Team Collaboration:

• Collaborate with other healthcare professionals, including physicians, therapists, and social workers.
• Work closely with office staff to coordinate and optimize patient care.

Qualifications:

• Current and valid Registered Nurse (RN) license in Florida.
• A minimum of 2 years of clinical nursing experience, preferably in homecare.
• Understanding in AHCA and CMS Medicare/Medicaid rules, preferably
• Understanding of clinical documentation, including OASIS, evaluations, orders, and documentation of care coordination.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a collaborative team.
