Duties and Responsibilities:

• Complete Initial HomeCare Assessments and Reassessments for clients

• Plan, Develop and Implement Plan of Care

• Evaluate Plan of Care effectiveness, modify as needed

• Quality Assurance

Required Qualifications:

• Current Georgia State LPN License

• Home Health/Home Care experience a plus

• CPR certified

• First Aid certified

• Annual negative TB
