Responsibilities:
- Directs and assists with resident, family/significant other education and notifies family on condition changes.
- Updates plan of care when needed and observes and documents observations and care given to the residents.
- Assists geriatric patients in Long Term Care (LTC).
- Collect samples for lab testing as assigned.
- Assists with formulating a plan of care for residents and provides nursing care to residents according to physician orders and regulations.
- Provide emotional and psychological support when needed.
- Assist ailing patients in daily necessary activities such as eating, bathing etc.
- Long-term care nurses also often assist patients with the tasks of their daily lives, including feeding, dressing, using the toilet and bathing.
- Administer injections, prescribed medications, enemas etc., clean and dress wounds and assist with other basic medical care tasks.
- Record a patient’s medical history accurately.
- Monitor patients’ condition including fluid intake and output and compose patient charts.
- Observe patients under treatment to identify progress, side-effects of medications etc.
- Take and record measurements of blood pressure, temperature, heart rate etc.
- Communicate with patients’ family or friends to provide advice, comfort and release instructions.
