Pathways Health and Community Support, LLC is Hiring a Care coordinator Near Springfield, OR

About your Role :

As a Foster Care Coordinators (FCC) you are responsible for the case management and oversight of youth placed in Maple Star certified therapeutic foster homes as well as the support of Family Care Providers (FCP).

You will have direct involvement in service planning, assessments, and weekly contractual contact with youth on your caseload to promote skill development and address youth behavioral challenges.

This position is also part of an after-hours / weekend on-call rotation. Additional compensation is provided for on-call work.

This position primarily serves youth and families in Marion, Linn, and Lane County.

  • Competitive pay starting at $22.00 / hour dependent on experience and education level
  • This position works from a home office and travels to meet youth in their foster home and in the community.
  • Additional pay available for on-call and bi-lingual services

Does the following apply to you?

  • A Bachelor’s degree with major study in social work, psychology, sociology or an allied field and have 2 years of experience in the care and treatment of children or young adults
  • A Master’s Degree with major study in social work, psychology, sociology or an allied field and have 1 year of experience in the care and treatment of children or young adults
  • Valid driver’s license, clean driving record and auto insurance
