Description

The Residential Caseworker, under the general supervision of the Case Manager Supervisor, will develop and nurture a therapeutic relationship with clients and their families; deliver all mandated casework services as described by the client’s service plan; plan program-driven related services; maintain appropriate communication with parent/legal guardian in coordination with supervisory and colleague staff (stakeholders); evaluate and report information relevant to treatment plan development and maintenance; were fit, coordinate, secure, evaluate, and report applicable social service information; develops, supports, and maintains linkages between the residence and outside social services in coordination with the treatment team.

MAJOR DUTIES AND RESPONSIBILITIES:

· Coordinate the delivery of required consumer services as outlined in the service plan.

· Manages a caseload of up to 9 Residential clients.

· Maintain all related service plan-related contacts in Connections and File Maker Pro. 

· Maintain compliance with the documentation requirements of the Office of Children and Family Services (case notes, service plans, family contacts, etc.) and the Administration for Children’s Services.

· Have familiarity with AFSA Federal Guidelines and ACS’s Methodology for the year’s respective year (IE, FY16 Methodology).

· Make phone contact with parent/legal guardians to ensure continued delivery of service plan goals and fidelity to client and guardian’s desired objectives.

· Participate in ensuring referral and support services are secured for families.

· Maintain contact with relevant outside social service agencies and mental health professionals in coordination with the treatment team.

· Ensure that the group homes policies and procedures regarding Child Abuse and Neglect are understood and implemented on behalf of all youth at Martin De Porres Youth And Family Services.

· Provide input on monitoring residents taking psychotropic medication.

· Provide feedback to Consulting psychiatrists for students taking psychotropic medication under the guidance and supervision of the Social Worker or administrative staff.

· Provide staff with ongoing collaboration around the relevant group and individual behavioral management needs, medication, and relevant personal, psychological, and family data that affects school/residential and community functioning.

· Provide administrative staff with linkage support around relevant system-wide issues in the student’s school and community/family milieu.

· Provide casework reports for relevant casework activities such as the court, or other advocacy services, including FASP and service plan reviews.

· Provide progress reports, ongoing updates, and discharge updates requested by the social worker or administrative staff.

· Provide support and presence when a resident requires emergency assistance (hospital, police, medical help).

· Provide contact with a resident’s family as often as required to ensure a defined treatment plan for each student.

· Ensure that Transitional Supports are in place when a resident leaves the Martin De Porres Youth And Family Services. 

· Assist with the Intake Screening process for new residents.

· Provide parent support as determined by the Clinical Director and Social Worker at a mutually agreeable location. 

· Perform all other duties assigned by the Martin de Porres Youth And Family Services Administration.

· The caseworker is under the supervision of the Clinical Director. Once a week, the caseworker will collaborate with program staff on activities and will be underline supervision during their program shift day. 

· The caseworker will be expected to inform the Clinical Director of any schedule changes or activities affecting their workweek and collaborate to maintain a schedule that meets the agency’s needs.

Requirements

· Ability to work with special needs residents with challenging, sometimes aggressive behaviors.

· Ability to work effectively with the parents/guardians of special needs students

· Ability to effectively advocate for youth in your care

· Ability to communicate effectively with staff 

· Ability to serve students beyond the confines of a regular workday

· Ability to visit parents/guardians beyond the geography of the residence

· Ability to maintain appropriate communication through phone contacts with parents.

· Ability to participate in Medical visits.

· Ability to develop and coordinate referral and support services for students and families.

· Ability to process intake paperwork.

· Ability to gather and maintain pertinent family and student information.

· Ability to develop and maintain case records.

· Ability to participate in direct resident milieu treatment.

· Ability to develop relationships with students, families, staff, and outside social service personnel. 

· Ability to speak English and Spanish is desired

· Willingness to participate in formative experiences with the residents (dining, social events, outings)

MINIMUM QUALIFICATIONS:

EDUCATIONAL:

· Bachelor’s degree in social work required 

LICENSE OR CERTIFICATION:

· Must possess a valid driver’s license with a good driving record

· CPR/First Aid

· Safe Crisis Management (SCM)

· Mandated Reporter

· Trauma-Informed Care

· LaSallian Culture of Care (LCC)

PREFERRED SKILLS:

· Satisfactory computer skills

· Excellent oral and written communication skills

· Excellent problem–solving capability and capacity to meet multiple deadlines under pressure

· Highly organized and detail-oriented

PHYSICAL REQUIREMENT:

· Lifting requirement of 50 Lbs.

· Ability to meet physical requirements of the position when performing the following: standing, walking, sitting, bending, stooping, squatting, kneeling, and climbing

· Physical agility and ability to react to emergencies, Including Safe Crisis Management certification as per regulatory guidelines

BACKGROUND VERIFICATION and CLEARANCE:

· Credential Verification

· State Central Registry Clearance

· Justice Center clearance (SEL)

· Medical clearance, which includes TB screening.
