Position: Case Manager Supervisor
Program: Home and Community Support Services for Individuals with Brain Injury
Location: Springfield Mo and surrounding counties.
Salary Range: $40,000 - $50,000

Job Description: We are seeking a highly motivated and skilled Case Manager Supervisor to lead our team in providing home and community support services to individuals with brain injuries.

Key Responsibilities:

• Provide direct services to clients in their home and community.
• Supervise and support direct care staff to ensure high-quality services.
• Develop and implement individualized care plans for clients.
• Provide direct services to individuals with brain injuries in their homes and communities.
• Transport clients in your own vehicle as needed.
• Oversee the daily operations of the home and community support program.
• Ensure compliance with all relevant regulations and standards.
• Provide training and professional development for staff.
• Coordinate with healthcare professionals, clients, and families to optimize care.
• Conduct regular assessments and evaluations of client progress.

Requirements:

• Bachelor’s or Master’s degree in a relevant field (Occupational Therapy, Recreational Therapy, Social Work, Psychology, Rehabilitation Counseling, Counseling, Rehabilitation Services, Education, Special Education, Nursing or closely related field.
• Additional qualifying experience may be substituted on a year-for-year basis for deficiencies in the required education, upon ABI Program Manager approval.
• Proven experience in a supervisory role within a similar setting.
• Strong organizational, communication, and leadership skills.
• Commitment to providing compassionate and client-centered care.
• Ability to transport clients in your own vehicle.
