As the Pharmacist, you will increase agency capacity to educate patients on their prescribed medications and coordinate with Primary Care Providers and specialists around new medications and changes consistent with professional standards and local, state, and federal law.

Job Duties

• Documents the results of discussions with doctors and patients, identifying current and potential medication treatment.
• Following proper pharmacy and regulatory safety procedures and standards pertaining to the 340B program, Americares, and GSCH sliding scale fee.
• Participates in regular personnel meetings and required meetings as necessary.
• Counsel PA /and or patients on meds consistent with NH Bop ruling.
• Participation in patient assistance and quality assurance programs as directed, ie..340B program, Americares, and GSCH sliding scale fee.
• Providing excellent customer service using best practices, i.e., Ensures prescriptions are processed accurately and in a timely manner to improve customer satisfaction.
• Perform DURS on all prescriptions or use the 5-point system during the final verification, review patient refill to ensure compliance.
• Maintains records of all transactions of the pharmacy as necessary to ensure compliance with local, state, and federal laws.
• Ensures uninsured patients receive the appropriate sliding fee based on household income.
• Assists in establishing and overseeing an in house 340 B program.
• Maintains inventory and stock levels consistent with pharmacy goals.
• Documenting and reporting medication errors to the pharmacist-in-charge.

Qualifications

• Current pharmacist licensure in the State of New Hampshire
• Current NH medical license.
• Current BLS Certification and/or Current ACLS or PALS certification
• Graduation from an ACPE accredited school of pharmacy
• 2 years professional experience
• Possesses in depth knowledge of pharmaceutical care processes
