Job Description

The Opportunity : As a registered nurse care coordinator, you will be responsible for the coordination of care for patients with various complex conditions, treatments, and care plans.

You will manage patient care through the continuum of care and ensure quality of care and services and will collaborate with a diverse multidisciplinary team to provide oversight of care, treatment plans and interventions.

To be successful in this position, you will need to have great communication skills, ability to develop relationships easily, strong teamwork and a passion for collaborating with diverse teams with different experience and backgrounds to ensure quality patient centered care is delivered.

You will perform as the patient’s advocate and will need to possess confidence in your skills to make sure decisions are made that are best for the patient.

Shift : Part time

The Work :

  • Makes quality patient outcomes a priority.
  • Overcomes obstacles to delivery of care through interdisciplinary coordination and communication.
  • Coordinates the interdisciplinary team and partners with physicians to ensure patient treatment plan progress.
  • Ensures that an appropriate plan of care is initiated.
  • Monitors care progression to ensure quality outcomes and effective transitions of care.

BSN Degree required within 3 years of start of position.
