SUMMARY: Coordinate the programmatic elements of Community Healing Project to increase access to mental health and wellness resources to address the impacts of past and present crime victimization and associated unaddressed trauma. 

This is a full-time grant funded position. This position is an administrative, outreach and collaborative client service-based position with a focus on increasing community access to mental health counseling and other victim services.

In coordination with Behavioral Health Outpatient Program staff, Family Services staff, and other Tribal service providers as part of a wellness team, the Services Coordinator will facilitate service coordination to address mental health and trauma. 

This program will develop and implement supportive services for crime victims who are not served by other Tribal programming, helping people to address trauma, and get needed services to recover and move toward wellness. 

The Services Coordinator will provide victim advocacy, connection to direct supportive services and referral to other supportive services as appropriate. Connecting clients to mental health services is a priority, and in that process, other victim support services may be identified. The Services Coordinator will help clients navigate these services and will follow-up with clients.

ESSENTIAL DUTIES AND RESPONSIBILITIES: 

The Service Coordinator will focus on case management, connection to services, and program outreach and evaluation:

  • Work closely with Tribal service-based staff teams to develop, implement and refine service delivery protocols and case management protocols for mental health clients and other community members who have experienced crime or trauma; 
  • Work closely with clients and tribal departments (Health Clinic, Behavioral Health, Family Services) to connect clients to services based on individualized recommendations and case management plan;
  • Work closely with Behavioral Health staff to support and refine the implementation of increased case management services in the community;
  • Work closely with Behavioral Health staff to ensure confidentiality, integrity and client-centered support as increased case management services are developed and integrated;
  • Assess and support clients to collaboratively build individualized wrap-around service plans; refer clients to services; follow up with clients and Mental Health Counselor; 
  • Work closely with clients and tribal departments (Health Clinic, Behavioral Health, Family Services) to connect clients to services based on individualized recommendations and case management plan; this may include home visits, attendance at appointments, follow-up support, and connection to other direct services, and community support groups;
  • Support client navigation through service, legal, or health systems and processes;
  • Collect and analyze data to measure the implementation and impact of program; 
  • Facilitate work sessions to determine any gaps in case management protocols and continue to implement and refine services coordination plan;
  • Research and update referral resource lists of internal and external resources available to clients; 
  • Solicit input from departments and outside partners on case management protocols and additional resources;
  • In coordination with the Family Services Director and with input from other tribal service-based departments, document, analyze and report on the program; 
  • Attend public community meetings as directed by Family Services Director to share information and educate community on trauma, mental health, and services
  • Provide community mental health education and information through direct services, community activities and media.
  • In collaboration with Family Services and BHOP staff, development outreach campaign materials to increase visibility of the new programming, decrease the stigma of accessing mental health and associated services, and invite community participation; develop promotional and informational materials to raise community awareness and literacy about mental health issues facing the community.
  • Attend trainings to support professional development in the role.
  • Other required duties: Maintain accurate records; Occasional travel to obtain further training; Other duties may be assigned.

  • Proficient in the use of the computer, Microsoft Word, Excel;
  • Collect data accurately and in a timely manner; 
  • Experience preparing educational materials such as posters, flyers and other communications media is desired; 
  • Experience working with Native American communities preferred.

LANGUAGE SKILLS: Ability to read, analyze, and interpret common scientific and technical journals, financial reports, and legal documents. Ability to respond to common inquiries or complaints from customers, regulatory agencies, or members of the community. Ability to write speeches and articles for publication that conforms to a prescribed style and format. 

MATHEMATICAL SKILLS: Ability to work with mathematical concepts such as probability and statistical inference. Ability to apply concepts such as fractions, percentages, ratios and proportions to practical situations. 

REASONING ABILITY: Ability to define problems, collect data, establish facts and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several concrete variables.

CERTIFICATES, LICENSES, REGISTRATIONS: Must have a valid Washington State Drivers License and be insurable under the Tribe’s plan. Must successfully pass a background check.

Requirements

 EDUCATION and/or EXPERIENCE: Master’s Degree in related field and 1 year of relevant experience OR Bachelor’s Degree in related field and 3 years of relevant experience, OR an Associate’s Degree in a related field and 5 years of relevant experience, OR a High School Diploma/GED and 7 years of relevant experience. Experience working with Native American communities is preferred.
