Job Description JOB TITLE: SOCIAL WORKER DEPARTMENT: ADMINISTRATION REPORTING TO: ADMINISTRATOR SUMMARY OF DUTIES: The Social Worker oversees bed utilization within the home and functions as an advocate for residents and is a member of the Leadership Team. Other duties may include but not limited to: liaise with families and residents to address concerns, provide tours of the home and liaise with the Regional Health Authorities, provides consultative and/or educational services to residents, families and staff on various topics. QUALIFICATIONS Bachelor's degree in Social Work from an accredited University and must be a member of the Manitoba Association of Social Workers. Strong interpersonal and communication skills that develop a positive work and living environment; Understanding of change management, organizational development with proven success; Solid understanding of Quality Improvement and Assurance programs Mentally and physically able to perform duties as outlined which includes ability to maintain regular attendance in addition to work in a very dynamic environment. Customer-focused attitude, with an emphasis on building and maintaining relationships with residents, their families, team members and the community Ability to generate sound, creative solutions and new approaches to daily challenges and which maximize the short and long-term goals of the facility. DUTIES AND RESPONSIBILITIES Assess applicant’s appropriateness to the facility by reviewing the application, consulting with facility staff, Long Term Care Access Center and/or hospital staff, visiting the resident and or family, making recommendations to the DOC/Designate on possible admission(s). Completes Vacancy Report and distributes to applicable recipients on a daily basis. Performs a guided and informed tour of the facility. Completes all pre-admission documents for nursing team and office and may complete admission agreement upon move-in with family and resident which may include the MDS demographic sections Welcomes new Resident and family to home and orientates them to same. Assesses the resident’s and family’s psychosocial needs and concerns on an ongoing basis, which may require assessments as per the Resident Care Manual Determines the need for social work intervention with the resident. In collaboration with the family and other disciplines, develops and implements a plan of intervention, reassesses and modifies plan as necessary which will be come part of the care plan; Provides social work services to residents and families from the point of application onward in relation to discharges. Utilizes team collaboration in all phases of the admission process. Advocates on behalf of the resident and ensures that social function is maintained Providing referrals to or assistance with participation in programs or services within the facility or through community agencies; Providing counseling and/or methods of planned intervention appropriate to the resident and family as necessary, i.e., crisis problem solving, bereavement counseling; Participating in the formation of support groups in which residents and families can maintain/develop/enhance their self-determination, life satisfaction and personal growth; Upon request, planning with resident/family for discharge to another facility, assisting in discharge arrangements, and insuring follow-up in the community; Work in line with the Extendicare Social Worker Manual Participates in the evaluation of care and/or service provided by the facility by developing, implementing and evaluation activities under the Quality Assurance/Improvement Program. Maintains all required records, reports, statistics, etc. for both residents and department in accordance with policies, procedures and legislative requirement. Liaison with Outside Agencies and Community: Promotes community awareness of Extendicare. Establishes and maintains rapport with personnel of the provincial health care associations and other sectors of the local health care community. Promotes education for staff awareness and sensitivity to the psychosocial needs of residents and to the emotional needs of families. In collaboration with the Education Service Department, conducts in-service training sessions related to the psychosocial needs of the resident population of the facility. Assumes responsibility for personal and professional growth, increasing professional knowledge in order to enhance skills. Performs all duties in a safe and efficient manner and maintains work area in a clean, orderly, and safe condition. Follows manufacturers’ instructions in the operation and use of all equipment and chemicals. Alert to potential health and safety hazards and takes appropriate action. Reports unsafe and hazardous working conditions and any equipment malfunctions. Knowledgeable of facility procedures for fire and environmental safety. Performs duties in accordance with prescribed sanitation and infection control procedures. Respects residents’ rights to dignity and privacy, and performs duties in a courteous and respectful manner. Adheres to “Standards of Employee Conduct,” “Commitment to Residents”, “Bill of Rights”, and “Extendicare Quality Care Standards” Manitoba Health Standards and Mission, Vision and Values. Works closely and collaborates with other staff and residents. Maintains good working relationships with other departments. Participates actively in interdisciplinary care conferences, committees, and meetings as required. Responsible for the development and upgrading of his/her knowledge and skills. Attends in-service sessions and conferences as required. Participates in facility Ethics Committee and activities and promotes ethics awareness to all staff.
