Support Coordinator/Case Manager to conduct assessments, coordinate services and monitoring of services for individuals with Intellectual Disabilities. Must have good communication skills both written and oral and be able to work with providers and families in a professional manner. Requires a Bachelor of Arts or a Bachelor of Science Degree, preferably in a Human Service field. Experience preferred but not required.
