Position Summary:
A Case Manager will engage patients and their families in developing a safe discharge plan. Case Managers are responsible for coordinating clinical care to include discharge planning, medical necessity, length of stay management. They will execute clear, complete and concise documentation of all interventions and assessments with providers, nursing staff, ancillary staff, patients, and families.
Required Education:
Bachelor's degree is preferred
Master's degree is preferred
Required License:
Registered Nurse or Licensed Practice Nurse in North Carolina is required (or compact state)
