Milestones Family Services is seeking an experienced, motivated, and highly organized professional to join our growing Adult Case Management program. We are offering full-time positions currently.
Case Managers work closely with a multidisciplinary team of professionals from State agencies (DHHS, OCFS, OADS), schools, home and community support agencies, medical providers along with any other support person the family and consumer choose to be part of the team.
The Case Manager assists consumers in accessing necessary services and support. The Case Manager assists in identifing the strengths and needs of the individual, develops, and coordinates a Person-Centered Plan (PCP), tailored to the consumer, makes referrals, advocates for services identified in the plan, monitors all progress, and evaluates the effectiveness of the services to ensure the consumer's needs and wants are being met. General Requirements:
Minimum one year of experience working with children, teens, or adults with intellectual or developmental disabilities (DSP, BHP, etc.)
Case managers must have reliable transportation, practice good time management, and have the skills necessary to facilitate 1:1 or large team meetings.
Adult Case Management experience preferred but not required.

In person at the office and travel to meetings, home visits and regular contacts with clients. There is some flexibility/availability for remote work.
Pay and Benefits:
· Hourly wage based on experience.
