Family Health Services is a non-profit, Federally Qualified Health Center (FQHC) providing quality, affordable health care to all. The Certified Family Nurse Practitioner provides medical services to the patients of Family Health Services in acute care and the clinical office setting in a holistic approach. We offer a competitive salary and benefits package including malpractice insurance coverage. Generous student loan repayment is also available through the NHSC Loan Repayment program, based on a 2-year full-time commitment.

Candidate must hold a diploma from an accredited School of Advanced Practice Nursing; licensed to practice as a Nurse Practitioner in the State of Ohio with current Nurse Practitioner Certification and a desire to serve the community of patients we serve. Experience with EMR is preferred. Recent graduates and newly licensed providers are welcome.
Essential Functions
The Certified Family Nurse Practitioner provides medical services to the patients of Family Health Services in acute care and the clinical office setting in a holistic approach. Essential Functions
1. Completes patient history and physical.
2. Documents all patient care provided according to policy and Standard of Care Agreement.
3. Formulates and directs plan of care for the patient as according to Standard of Care Agreement.
4. Refers to other physicians, specialists as needed.
5. Prescribes medication in accordance with Ohio Revised Code and Ohio Administrative Code and Formulary provided by the Advanced Practice Nursing laws.
6. Complies with State Advanced Practice Nursing Guidelines/Law on Practicing Medicine.
7. Responds to medical emergencies.
8. Directs patient education for health education, maintenance, emotional and physical needs.
9. Participates and provides care as according to the chronic disease model.
10. Participates in Medical Staff activities at FHS.
11. Participates in Performance Improvement/Risk Management Program.
12. Utilizes supplies in a cost-effective manner.
13. Demonstrates effective communication skills.
14. Establishes and maintains professional working relationship with other members of the health care team.
15. Participates in planning and problem solving.
16. Participates in continuing education and professional growth.
17. Attends provider’s meetings.
18. Maintains knowledge of, and complies with, personnel and medical, policies and procedures.
19. Assists other team members.
20. Assists with seasonal programs as needed.
21. Serves as a role model for clinical staff.
22. Adheres to the standards and policies of the Organizational Privacy/Security and Compliance Programs, including the duty to comply with applicable laws and regulations (HIPAA, OSHA, OIG, guidelines, other State and Federal laws). This also includes reporting to the Board of Directors, Compliance Officer, Privacy Officer, supervisor, suggestion box, any suspected unethical, fraudulent, or unlawful acts or practices.
Required Education and Experience
1. Must hold a diploma from an accredited School of Advanced Practice Nursing.
2. Licensed to practice as a Nurse Practitioner in the State of Ohio.
3. Current Nurse Practitioner Certification.
Preferred Education and Experience
1. Nursing experience in ambulatory care setting, community health setting is ideal.
2. Minimum 3 years’ experience in practice as a nurse practitioner.
