Job Overview:

We are looking for a compassionate and highly organized Patient Care Coordinator to join our healthcare team. The ideal candidate will be responsible for managing patient care activities, ensuring that patients receive the highest quality of care and support. Key Responsibilities:

  • Patient Liaison: Serve as the primary point of contact for patients, addressing their needs, concerns, and inquiries in a timely and empathetic manner.
  • Care Coordination: Coordinate patient care activities, including scheduling appointments, follow-up visits, and referrals to specialists or other healthcare services.
  • Patient Education: Provide patients with information and resources about their treatment plans, medications, and self-care practices to ensure they are well-informed and engaged in their care.
  • Administrative Support: Manage patient records, update electronic health records (EHRs), and ensure accurate documentation of patient interactions and care plans.
  • Collaboration: Work closely with physicians, nurses, and other healthcare providers to ensure seamless and coordinated patient care.
  • Quality Assurance: Monitor patient care processes and outcomes, identifying areas for improvement and implementing changes to enhance the quality of care.
  • Follow-up Care: Conduct follow-up calls and check-ins with patients to monitor their progress, address any issues, and provide ongoing support.
  • Insurance Coordination: Assist patients with insurance-related matters, including verifying coverage, obtaining prior authorizations, and handling billing inquiries.
  • Patient Advocacy: Advocate for patients' needs and preferences, ensuring they receive appropriate care and support throughout their healthcare journey.

Qualifications:

  • Education: Associate's or Bachelor’s degree in healthcare administration, nursing, social work, or a related field.
  • Experience: Minimum of 2-3 years of experience in patient care coordination, case management, or a similar role within a healthcare setting.
  • Skills:
  • Strong understanding of healthcare processes and patient care management.
  • Excellent organizational and multitasking abilities.
  • Superior communication and interpersonal skills.
  • Proficiency in electronic health records (EHR) systems and other healthcare technology.
  • Empathetic and patient-centered approach to care.
  • Ability to work independently and collaboratively in a team environment.
  • Certifications: Certification in case management or patient care coordination is a plus.
