Candidate needs to reside in the west or central region of Texas as they will be supporting members in this region both telephonically and in-person to fulfill job duties.
Position Purpose: Responsible for assessing needs and developing service plans for members in accordance with their goals and preferences in regards to employment, housing, participant direction, independent living, and/or community transitions. Serve as a subject matter expert for Service Coordinators and Care Managers in supporting the development of person-centered service plans for members.
Education/Experience: Bachelor’s degree in related field or equivalent experience. 2 years’ experience in employment, housing, transitions, or participant direction programs. Experience working with LTSS, chronic or complex condition populations preferred.
Licenses/Certifications: Must have a valid State Driver’s License
Pay Range: $25.97 - $46.68 per hour
