Are you a Registered Nurse and looking to join a dynamic team serving the children of Rowan County? Salisbury Pediatric Associates, the leader in pediatric care in the region, is looking for an experienced Care Manager to join our team as we focus on improving the health of all children. The Care Manager's role will be the key piece in our plan to address the needs of our pediatric population by assessing, planning, implementing, coordinating, monitoring, and evaluating the options and services available to promote quality, cost-effective health outcomes for our children. In conjunction with Community Care of NC, we will work together to achieve the best outcomes.
Job will include but not limited to:
Formulates and implements a care management plan that addresses the patients identified needs by assessing the patient and family needs, issues, resources, and care goals; determining the choices available to individual patients; educating the patient/family on the choices available
Create care plans and comprehensive assessments for specified risk patients related to care management needs or SDOH
Reviews utilization and quality reports routinely and identifies gaps in care who may need additional care management support
Maintains CMIS portal databases on care managed population. Maintains accurate and timely documentation of CMIS portal
Performs follow up calls for patients recently discharged from acute hospitalizations or transitional care and who are considered high risk for readmission
Assists and supports ADT (admission, discharge and transfers) notifications and providers appropriate follow up and education
Performs medication reconciliation for all high risk patients enrolled in care management
Promotes patient self-management and empowers patients and families to achieve maximum level of wellness and independence. Interacts professionally with patient and family and involves them in formation of care plan
Works closely with health care time in development of care coordination or care plan based on psychosocial needs
Acts as a resource for other members of the health care time by providing in service education and consultation in understanding the clinical and medical factors related to health needs
Maintains knowledge of the field including Medicaid and other third-party reimbursement sources
Participates in planning and implementing quality improvement activities
Generates reports based on NCIR data, immunizations, EHR metrics, well care appointments and other quality report measures
Performs all duties and responsibilities within scope of practice identified by NCBON (North Carolina Board of Nursing)
Participates in onboarding and orientation of new team members. Precepts and mentors team members and promotes collaborate teamwork
Maintains minimal license degree of RN (Associate of Nursing or Bachelor of Nursing)
