What We’re Searching For

Hometown Home Health is seeking a dynamic individual to join our team in a LPN position. This team member will play a major part in service to our patients and maintaining an excellent standard of care. 

What You’ll Do

• Follows the plan of care to provide safe, competent care to the client.
• Assist with the preparation, implementation, and continuing evaluation of the patient care plan.
• Assist the therapy personnel with rehabilitative processes.
• Help teach the patient the appropriate self-care techniques.
• Prepare clinical and progress notes.

What You Need

• Graduation from an accredited School of Practical Nursing.
• Must be licensed in the state of Florida as a Licensed Practical Nurse
• One, or more, years in the Home Health Agency or in a hospital setting. Home health experience preferred.
