As a MDS Coordinator you will be responsiblefor coordinating MDS (Minimum Data Set) as required per State & Federalguidelines. You will also provide residents with direct care to assure qualityof life in a home like environment while offering additional support to familyand visitors. You will maintain a professional behavior while functioning as aninterdisciplinary team member.

Qualifications:

• Graduate of accredited school of nursing (LPN) with current state license.'
• Must be LPN and have experience in MDS 3.0 and skilled nursing.
• One (1) or more years clinical experience SNF preferred
• Prefer 2 or more years in long term care facility or assisted living
• Knowledge of Medicare, RUGS's state and federal regulations.
