DMC Empowering Health Services is Hiring a Rehabilitation Specialist Near Rosedale, MD

DMC Empowering Health Services is a growing Psychiatric Rehabilitation Program. We provide services to children and adults in Baltimore. Our number one goal is to help individuals thrive at their highest potential. We are looking for a Rehabilitation Specialist that shares our mission and vision of becoming a better Baltimore.

Job Description: The Rehabilitation Specialist is responsible for assisting direct service staff facilitate interventions that results in the improvement in symptoms or personal distress for clients. The rehab specialist is responsible for assisting the client and the client’s treatment team to address mental health and somatic issues such as mental illness, substance abuse or behavioral issues in a secure and safe environment. This position will provide services to both children and adults and assisting with providing behavioral interventions through community outreach, individual, family, and group counseling services. The rehab specialist is also responsible for developing person-centered Individual Rehabilitation Plans, clinical assessments and maintaining record documentation that is consistent with accreditation standards and COMAR requirements and regulations.

Job Duties and Responsibilities:

  • Provide individual and group counseling/supports to clients and family members
  • Collaborate with community therapists and other mental health professionals to coordinate client care and provision of services
  • Provide strategies in designing planned interventions for complex clients by providing feedback to the treatment team
  • Assist Direct Service workers with providing services aimed at reducing symptoms/behaviors associated with a mental health disability
  • Assist clients in educational, work skills, sobriety or with other issues related to their diagnosis by obtaining skills to live independently or by preventing decompensation or a more restrictive situation.
  • Provide training on mental health related issues to other staff responsible for delivering services
  • Develop treatment plans in conjunction with direct care staff
  • Provide assessment of clients functioning and strengths in the following areas; independent living, housing, employment, management of medications, mobility and transportation, social relationship and leisure activities, educational and vocational training, resources and adaptive equipment, current resources and support system, legal status, and behaviors that are possibly dangerous to the individual or others.
  • Conduct educational/process group sessions including program curriculum and life skills groups
  • Work with clients to develop individualized service/treatment plans with measurable goals including substance abuse treatment, relapse prevention, referrals and reporting to appropriate parties
  • Assist in the intake, assessment and follow up to ensure clients/clients adherence to treatment plan or court ordered treatment.
  • Plan and implement recreational activities for clients and their families

Participate in case conferences and treatment team reviews of client’s progress

  • Attend in service trainings, community forums and educational sessions

Provide general administrative support, as needed (i.e., program reports)

Provide client monitoring and case management

  • Assist in community outreach and referrals, and client follow up
  • Establish community relations and linkages with other service providers

Minimum Qualifications:

  • Licensed mental health professional that includes certification for Rehabilitation counselor that is currently certified by the Commission on Rehabilitation Counselor Certification; or bachelor’s prepared certified psychiatric rehabilitation practitioner currently certified by the U.S. Psychiatric Rehabilitation Association
  • Work Remotely
  • Temporarily due to COVID-19

Job Type: Full-time

Pay: $60,000.00 - $70,000.00 per year
