POSITION SUMMARY
The Primary Care nurse is a professional Registered Nurse who provides high-quality, evidence-based care for patients who are cared for in the Primary Care Medical Home setting within a fixed clinical setting or Mobile Van. They perform assessment, triage, case manage, and perform Chronic Care disease management. They must be able to promote a safe environment for all patients including those with altered sensorium due to medication or an acute or chronic illness. Nurses are to document using appropriate forms, follow protocols, and guidelines, adhering to policies and evaluate resources. The RN effectively implements patient activities towards health promotion and disease prevention, assisting the patient in attainment of optimal levels of function through, direct care, as well as provision of supportive measures as appropriate.
Based on education/experience, the RN may be assigned responsibility as a preceptor to new staff/trainees.
GENERAL EDUCATION REQUIREMENTS
Graduate from a program accredited by the National League for Nursing Accrediting Commission (NLNAC), or the Commission on Collegiate Nursing Education (CCNE) and has met licensure requirements for practice.
Must have an Associate Degree in Nursing, Bachelor of Science in Nursing, preferably from an accredited university.
ADDITIONAL QUALIFICATIONS
•Clear and active Florida/compact Nursing License
•Minimum of four (4) years nursing experience with two (2) years recent experience in an outpatient/ambulatory care setting preferred
•Current BLS/CPR certification from the American Heart Association or the American Red Cross
•Proficient in Electronic Medical Records (EMR)
•Demonstrates strong oral and written communication skills
•If a current clinical BHA employee earns a registered nurse license during employment, the experience qualification requirement may be waived based upon leadership discretion.
ROLE
The Registered Nurse (RN) is responsible for the global oversight of Primary Care Medical Home team members to achieve desirable patient outcomes through day-to-day and/or telephonic case management. The RN assists with the development, assessment, implementation, and monitoring/evaluation of comprehensive plans of care to ensure efficient, effective, and coordinated care in meeting and promoting standards of wellness. He/she coordinates an interdisciplinary approach to assure physical, psychosocial, and environmental needs and resource delivery to achieve patient goals, The RN relates in an open, friendly manner, shows interest in others, and initiates and develops interdisciplinary relationships for a broad spectrum of resources. He/she acts as an advocate seeking and coordinating solutions for self and members of the team. The RN adheres to established policies, standards of care, and standards of practice; demonstrates leadership when providing nursing care; and serves as a role model and team leader.
The RN is also responsible and accountable for demonstrating the clinical, educational, experiential, and knowledge/practice-based competency requirements.
PRIMARY ACCOUNTABILITIES
•Uses the nursing process to formulate safe and effective care based on each individual’s unique needs (Including psychosocial, spiritual, cultural, biophysical, educational, and age specific needs)
•Maintains clinic nurse and providers schedules to ensure timeliness
•Provides comprehensive and coordinated nursing care to scheduled patients. Collaborates with services internal and external to BHA to effectively meet the health promotion/disease prevention, acute, chronic, and long-term needs, based on the plan of care
•Coordinates, delegates, and directs clinical personnel.
•Prioritizes care based on patient/family needs, available resources of time, personnel, supplies, and equipment
•Documentation is timely, accurate, and concise.
•Effective communication with patient and family members Involves patient/family in care decisions and provides education based on identified needs.
•Effective working relationships with all health care team members, patient, and family members.
•Protects and respects patients’ rights and confidentiality.
•Performs daily duties and quality controls including but not limited to:
•Medication inventory
•Point of care testing quality controls
•Medication refrigerator temperature
•Stocking supplies
•Checking for expired supplies and medications
•Uses self-directed learning to acquire and maintain current knowledge in nursing practice.
•Participates in development of clinical goals, standards, protocols, policies, and scheduling.
•Performs competent care for patients using the primary care medical home model, specific duties include but are not limited to:
•Triage non-scheduled patients
•Track and manage patient’s paperwork
•Place referrals and order lab/ diagnostic testing according to protocols
•RN to cover MA/LPN duties when needed
•Call back patients as indicated
•Participates with huddles with primary care team
•Performs clinic or telephone triage within established standards. Notifies or consults with Provider, as needed; documents in EMR
•Maintains daily patient flow to ensure patients will be seen as soon as possible using all available resources; checks patient in and assesses their needs as stated in the scheduled appointment
•Enters appropriate patient history into medical record
•Provides patient, family and caregiver education based on medical and/or nursing diagnosis, which includes unique age-related needs and arranges for follow-up educational resources, as needed
•Manages and maintains the delegated critical line. All calls should be returned or delegated within 2 hours to determine appropriate medical follow up; issues should be resolved within 24 hours
•Assists with indigent medication program, as applicable
•Enters dispensed medication onto excel spreadsheet and reconciles pharmacy inventory as applicable
•Ensures pediatric patients are up to date on all immunizations and are entered correctly in the EMR and Florida Shots
•Maintains lab Quality assurance (QA) as needed, and ensures QA’s are being done appropriately
•Documents initial and ongoing patient assessment
•Completes patient medical history within established standards; verifies chart for completeness.
•Enters all applicable data into EMR
•Enters check in and checkout times in EMR as applicable
•Able to perform and carry out projects as delegated by the Site Manager or Director of Clinical Operations or designees
•Takes ownership and represents Brevard Health Alliance’s mission and organizational goals established by the executive committee. These include, but are not limited to: Productivity, Quality, Patient Satisfaction, and Employee Retention. It is the duty of each BHA employee to incorporate these goals into all daily work routines and strive to achieve and surpass all goals by working as a team. The mission and goals of BHA will be incorporated into each associate’s performance plan to be evaluated annually.
•Travel within different BHA clinics and will reimburse your local travel, per BHA policy and the current IRS reimbursement rate.
