We are currently seeking a compassionate and dedicated professional to join our team as a Full Time RN Clinical Supervisor serving our Beaufort Branch including Beaufort, Jasper, Colleton, Hampton, Barnwell, Bamberg and Allendale Counties.

*Position Summary/Primary Responsibilities:
The Clinical Supervisor is a registered nurse who manages the coordination, supervision, and implementation of Professional and supportive services to clients in a safe, cost-effective manner. The Clinical Supervisor is responsible for the coordination of care during the episode of health care treatments. The Clinical Supervisor will provide education and training related to clinical practice issues and regulation and reimbursement changes with new employee during precepting phase of new hire orientation.

Minimum Qualifications/Requirements:
 Minimum of an Associates Degree in Nursing, BSN preferred.
 Licensed as a Registered Nurse, in good standing in respective state
 Current CPR certification required.
 National Certification in Hospice and Palliative Nursing preferred
 Minimum of three to five (3-5) years of clinical Registered Nurse experience required
 Minimum of one to three (1-3) years of leadership experience required
 Minimum of one to three (1-3) years of hospice experience required
 Strong working knowledge of state and federal hospice regulations
 Must possess ability to motivate teams and to produce quality materials within tight timeframes and manage several projects simultaneously.
 Must be proficient in technology systems including Microsoft Office and Electronic Medical Record platforms.
 Must be able to read, write, and communicate verbally in the English language.
 Valid driver’s license and auto insurance required.
 Willingness to travel as required.
 Excellent verbal and written communication skills.
 Strong sense of urgency and personal commitment.
 Conflict resolution skills and the ability to promote, motivate and effect positive changes
 On Call Requirement
 May be required to function as Hospice Administrator
 Reliable transportation

Essential Functions: (The following is a list of responsibilities of this position but is not intended to cover all duties that this position may be required to perform from time to time.)
1. Plans, directs, and participates in delivery of professional services to hospice clients. Participates in the organization's strategic planning activities. Focuses services toward diagnosis- specific client care needs
2. Communicates significant findings, problems, changes in condition or environment to the physician and other personnel involved with patient care.
3. Reports unsafe conditions to the physician as appropriate.
4. Initiates plan for patient safety, using the patient, family, and community resources.
5. Demonstrates knowledge in all levels of hospice practice.
6. Manages and directs a team of Field staff, to ensure the delivery of safe, effective, and appropriate hospice services.
7. Coordinates and directs staffing to promote quality services for clients.
8. Coordinates services to ensure the availability and competency of staff to respond to agency referrals
9. Identifies deviations from normal Nursing and normal patient condition status. Initiates appropriate actions based on the deviation.
10. Provides education and training related to clinical practice issues and regulation and reimbursement changes.
11. Treats patients and caregivers in the highest and most effective manner to attain leading market patient satisfaction scores.
12. Facilitates admission reviews and case manages for a team of patients.
13. Establishes and implements priorities for services within the limitations of available resources
14. Coordinates on-call services
15. Meets with nursing & other clinical staff on a regular basis to provide guidance and information related to specific issues
16. Supervises the coordination of services with providers in the delivery of care and related issues.
17. Process and coordinate patient referrals, physician orders, staff assignments, and patient re- certifications, and communications between various caregivers, the patient and their family, and the referring physician
18. Verifies the plan of care for the episode according to physician's orders, assessment data, and established standards and guidelines
19. Discusses and directs the field staff assignments for patient care. Compares the Aide care plan to the actual care given as verified with patient/caregiver.
20. Directs and assures office protocols are followed and the office in its entirety is clean, orderly, and productive.
21. Instructs and evaluates the field staff with special patient care procedures in accordance with the scope of practice and hospice care policies of the agency
22. Identifies and instructs staff on admission criteria for new clients and evaluates eligibility for hospice care and oversees review of records to ensure continuing hospice client eligibility for covered services
23. Oversees the office support staff and reviews productivity daily
24. Assures compliance with the requirements of state licensure, Medicare certification, and any other applicable oversight agencies through policy and procedure development, staff education, and ongoing monitoring activities
25. Recommends new policies and/or changes in current policies to improve quality of client care and efficiency/effectiveness of services.
26. Promotes compliance with all fiscal intermediary and/or other third party payers, through education, coaching, and other assistance as necessary.
27. Keeps fully cognizant of current Medicare, Medicaid, and third-party reimbursement regulations and changes and ensures compliance
28. Supervises and provides direction to agency personnel in an effort to ensure quality and continuity of services
29. Participates in the hiring and orientation of personnel
30. Identifies the standards of practice for agency personnel and communicates these standards to staff.
31. Manages the day to day operations of the branch and seeks input from the regional director as needed.
32. Provides staff development and in-service education programs in collaboration with the education department to ensure competencies of staff and to meet regulatory requirements
33. Communicates effectively with all members of the interdisciplinary team.
34. Interprets and enforces human resource policies and procedures in a consistent manner
35. Conducts performance evaluations in accordance with agency policy
36. Monitors employee turnover, overtime and absenteeism. Takes appropriate actions to address problems/issues
37. Collaborates with the Regional Director and other clinical management staff to ensure proper staffing of qualified, competent staff
38. Administers, coordinates and evaluates operational Systems to enhance Operations and internal delivery of quality patient care
39. Assists quality improvement teams with data collection and documentation review for the Performance Improvement plan established by the agency.
40. Assists with medical community education and discharge planning by participating in departmental meetings.
41. Flexible to exceed patient and referral source needs.
42. Identifies and implements changes in clinical and/or operational practice based on the findings of the Performance Improvement program.
43. Pursues ongoing education to remain current with all aspects of profession
44. Facilitates problem-solving sessions with nursing and all staff applicable to resolve client and/or employee issues.
45. Stays current on the available community resources, health care costs, and industry trends through self- education and access to outside educational opportunities
46. Administers, coordinates and evaluates operational Systems to enhance Operations and internal delivery of quality patient care
47. Responsible for overall financial performance of designated branch/hub location
48. Maintains current knowledge of and promotes compliance with all federal, state and local laws related to data integrity and HIPAA
49. Assures compliance with requirements of state for Medicare certification through policy and procedure development, staff education and mentoring activities.
50. Assures safety and office compliance of fire, OSHA, MSDS needs as per policy.
51. Acts as a liaison between agency and other subsidiaries. Provides written and verbal communication within the organization. Responds to and resolves internal customer concerns. Seeks opportunities to collaborate with other subsidiaries in providing a seamless continuum of care to clients of the system.
52. Demonstrates personal responsibility with regards to discretionary and confidential material.
53. Communicates information to staff about available support services, resources, agency policies and procedures and organizational plans
Investigates complaints and problems and ensures problem resolution and correction of deficiencies and reports to Supervisor.
54. Interacts with agency staff in an engaging and effective manner to retain high performers while holding all accountable to performance standards.
55. Participates in the hiring following legal interviewing procedures and ensures a thorough and effective orientation of personnel to ensure retention and engagement of new hires.
56. Communicates effectively with all members of the interdisciplinary team, ensuring strong two-way communication and effective resolution of employee concerns and questions that arise.
57. Focuses on issues that cause voluntary employee turnover and takes appropriate act
58. Conducts effective performance evaluations and provides for the ongoing individual development and succession of high potential staff.
59. Collaborates with HR on all corrective coaching and disciplinary actions involving staff.
60. Attends assigned in-service programs, training, education, seminars, or other means of learning as permitted by supervisor.
61. Written and verbal communication is expressed clearly and in a cooperative manner.
62. Participates on committees and task forces as requested.
63. Provides positive, supportive communication to physicians, patients, families, visitors, and other agency personnel.
64. Promotes Agency image by adhering to the dress code and reports to work as scheduled.
65. Maintains availability by phone as directed by company policy.
66. Meets minimum productivity requirements based on agency standard.
67. Abides by company policy and procedures as outlined by company policy manual.
68. Performs other duties as assigned by the Supervisor.
Physical Requirements/Working Conditions:
1. Complete required work daily.
2. Cope with work involving ill, disabled, and dying patients.
3. Sit, stand, bend, lift, and move intermittently.
4. Move equipment and supplies necessary for job functions.
5. Read, speak, and write English to fulfill job functions in an understandable manner.
6. See/hear or use prosthetics that enable accomplishment of job.
7. Meet the health requirements of the agency.
8. May have possible exposure to infectious diseases.
9. Abide by all occupational safety and bloodborne pathogen protocol to minimize illness and/or injury.
10. Move, support, and/or transfer patients as required to perform essential functions of position.*
