Responsibilities
Schedule patients to branch field clinicians. Communicate with field staff, patients, physicians, referral sources, caregivers, and other service providers in order to maintain proper care coordination and continuity of care.
Manage the on-call notebook and hospitalization logs to enhance communication among stakeholders.
Qualifications
or 2) have at least one year of home health, hospice, or pediatric experience within the last 24 months, and have a demonstrated understanding of staffing and scheduling requirements related to home care services.
Must have basic demonstrated technology skills, including operation of a mobile device.
Education and experience, preferred
Previous experience in home health, hospice, or pediatrics is preferred.
